HOW I DO IT Modi®ed Technique for Esophagojejunostomy After Total Gastrectomy

چکیده
Gastric cancer is worldwide one of the most common causes of cancer death. Total gastrectomy is the gold standard treatment for gastric carcinomas originating in the gastric body or fundus. There have been important decrease in surgical morbidity and mortality after total gastrectomies. However, some major complications are related to the surgical technique. Esophagojejunostomy leaks are serious complications that, if not recognized and managed expeditiously, can be life-threatening. Esophagojejunostomy is the most common type of reconstruction after total gastrectomy but there are several ways to perform it [1±6]. The authors describe a modi®ed technique for an easier esophagojejunostomy after total gastrectomy.

Minimum leakage rate (0.5%) of stapled esophagojejunostomy with sacrifice of a small part of the jejunum after total gastrectomy in 390 consecutive patients.
BACKGROUND The development of new surgical instruments and devices has facilitated the performance of esophagojejunostomy after total gastrectomy. However, total prevention of dehiscence of anastomoses remains difficult. We introduced a new procedure for esophagojejunostomy using a circular stapler, requiring sacrifice of only a small part of the jejunum. METHODS The study group comprised 390...
متن کاملPouch Roux-en-Y vs No Pouch Roux-en-Y following total gastrectomy: a meta-analysis based on 12 studies
After a total resection of the stomach, the continuity of the gastrointestinal tract can be restored either by Roux-en-Y esophagojejunostomy with or without a pouch. There is still no consensus on the best reconstruction technique. The aim of this report was to derive a more precise estimation of Roux-en-Y esophagojejunostomy with a pouch compared with Roux-en-Y esophagojejunostomy without a po...
متن کاملImpact of stepwise introduction of esophagojejunostomy during laparoscopic total gastrectomy: a single-center experience in Japan
BACKGROUND The number of laparoscopic gastrectomies performed in Japan is increasing with the development of laparoscopic and surgical instruments. However, laparoscopic total gastrectomy is developing relatively slowly because of technical difficulties, particularly in esophagojejunostomy. METHODS We retrospectively reviewed 83 patients with early gastric cancer in the upper portion of the s...
متن کاملSafety of intracorporeal circular stapling esophagojejunostomy using trans-orally inserted anvil (OrVilTM) following laparoscopic total or proximal gastrectomy - comparison with extracorporeal anastomosis
Background: There have been several attempts to develop a unique and easier way to perform esophagojejunostomy during laparoscopy-assisted total gastrectomy or laparoscopy-assisted proximal gastrectomy. The OrVilTM system (Covidien, Mansfield, MA, USA) is one of those methods, but its technical and oncologic feasibility have not been proven and need to be observed. Methods: Among 87 patients wh...
متن کاملAn automatically contamination-avoiding technique for intracorporeal esophagojejunostomy using a transorally inserted anvil during laparoscopic total gastrectomy for gastric cancer
BACKGROUND Intracorporeal Roux-en-Y esophagojejunostomy during laparoscopic total gastrectomy for gastric cancer remains a challenging manipulation due to the uncontrolled direction of the jejunal side or unintended embedded tissues, although several methods have been introduced. In this study, we simplified the procedure based on a surgical string fixing technique using a transorally inserted ...
